Provides professional, supportive counseling and case management services to clients who are affected by severe and prolonged mental illness and/or substance abuse addiction.
Provides a full range of case management and supportive services to clients: home based services; mobile outreach; client advocacy; maintenance of basic living needs; assessment of benefits; and linkage of services.
Works with Program Team Leader to provide immediate access to services.
Demonstrates willingness to work collaboratively and provide cross team and cross building coverage.
Conducts psycho-social evaluation and treatment by: providing direct client care that is consistent with the individual support plan and treatment plan; providing on-going support to clients; and providing crisis stabilization services as needed.
Updates information on the clinical assessment as required and within the scope of license or MHRT certification.
Provides collateral communications/consultation to family members and/or caregiver, or other community providers and makes referrals as needed.

Education Requirements
Bachelor's Degree in Social Work or related field, or a RN who holds a valid license to practice in the State of Maine required.
Maine State licensure to practice as LSW is preferred.
MHRT/C certification or eligibility to receive certification is required.
One to two years prior experience working with specific client population to which this position is primarily assigned.
Two to three years prior experience working with a broad spectrum of clients is preferred.
Prior expereince with clinical methods and techniques used in supportive counseling as related to mental health issues and substance abuse is preferred.
Prosses valid driver's license and safe driver's record.
