Case Management - working to creatively find ways to connect individual and families/support systems with needed resources and helping them navigate the often-confusing system.
 

An exciting part-time (20 Hour/Week) opportunity for an individual to serve in a dual role as Children’s Case Manager & Community Integrations Services Case Manager is available in the Windham area.

 

In the capacity of Children’s Case Manager you will work with children and families performing assigned intakes/assessments, planning, monitoring and evaluating child/family services to include coordination of and advocacy for services.  Meetings can take place in the home, school, or at other local sites.     

 

In the capacity of Community Integrations Services Case Manager you will work with adults living with chronic and severe mental health issues in order to foster empowerment and independence.  Your work with consumers in their homes and within the community will focus on providing the least restrictive level of care that supports recovery.
 
Potential for hours to increase as program needs shift!
 
Meeting the needs of the consumer and their families through:  assessment, coordination,
 networking, advocacy, linkage, crisis planning, behavior planning!

Job Requirements

In serving this unique dual role experience working with children with special needs and experience working with adults with major mental illness is required.  The ability to adapt to consumer and family/support system needs is critical.  Strong organizational skills, computer proficiency, and valid driver’s license required.

Education Requirements

Dual credentialing requirements must be met and maintained:  OQMHP eligibility and MHRT/C eligibility.  A bachelor’s degree in a mental health related field along with one year case management experience typically satisfies these credentialing requirements.
