Our Targeted Case Management (TCM) program is GROWING!
Our TCM program services children ages birth to 20 years old that have a mental health diagnosis or an developmental disability and are in need of assistance with advocacy, case coordination, assistance with applications, accessing resources, and making referrals.
We have an opening for a part time Children's case manager with a high probability that it will become full time as the caseload grows in our new Biddeford location.
Some of the job expectations are as follows:
Attending all client centered appointments
Assist in advocacy for the needs of clients
Complete all required documentation timely from intake assessments to treatment plans and CANS and APS reviews
Develop and initiate crisis plans when needed
Coordinate transportation and occasionally provide transportation to mental health appointments
Maintain professional communication with families and other professionals.
The position will be based out of the Biddeford office and will travel throughout the service area.
Ideal applicants will have a positive attitude, strong work ethic, strong communication skills, and an ability to work both independently and as part of a team.
Must be able to travel within the service area.
Bachelors degree in a human services field with an active LSX or LSW licensure.
