Our Targeted Case Management (TCM) program is GROWING!
Our TCM program services children ages birth to 20 years old that have a mental health diagnosis or an developmental disability and are in need of assistance with advocacy, case coordination, assistance with applications, accessing resources, and making referrals.
We have an opening for a part time Children's case manager with a high probability that it will become full time as the caseload grows in our new Biddeford location.
Some of the job expectations are as follows:
  • Attending all client centered appointments
  • Assist in advocacy for the needs of clients
  • Complete all required documentation timely from intake assessments to treatment plans and CANS and APS reviews
  • Coordinate services
  • Develop and initiate crisis plans when needed
  • Coordinate transportation and occasionally provide transportation to mental health appointments
  •  Maintain professional communication with families and other professionals.

    Location Notes:The position will be based out of the Biddeford office and will travel throughout the service area.
    Job Requirements
    Ideal applicants will have a positive attitude, strong work ethic, strong communication skills, and an ability to work both independently and as part of a team.

    Must be able to travel within the service area.
    Education Requirements
    Bachelors degree in a human services field with an active LSX or LSW licensure.
