The Practice office employee is responsible for the facilitation of the flow of the activity of health team members, patients, physicians, and people accompanying patients to the office.
Front Desk personnel perform all clerical functions related to the office, answer the phones and accurately document correspondence within a medical record telephone encounter, transcribe physicians' order, schedule patients, greet patients, and modify patient demographic/insurance information to ensure its accuracy. Medical Record Responsibilities: According to our policies and state guidelines for Release of Information, staff responds promptly to patient requests for medical record information. Responsibilities also include timely scanning of confidential clinical/general information into the designated area of the medical record and re-assignment to appropriate medical personnel.
As outlined below the Practice Office Staff person may or may not also have secondary duties as applicable to his/her specific office
Coding/Charge Entry Responsibilities:
Coding/Charge Entry personnel transcribes physician charge information and enters the diagnosis and procedure codes within the billing system, validate accurate patient insurance information, and work in partnership with patient accounts to reconcile billing requests and/or input of billing modifications.

The PCMH is a health care setting that facilitates partnerships between individual patients, their personal physicians, and the patient's family or support system. In the team-based care model, all care team members contribute to the health of the patients by working at the top of their licensure and/or skill set.
Education: High School Diploma or equivalent, computer skills, medical secretary's courses including medical terminology preferred. One year experience in a hospital environment, physician's office, third party client relations, or patient accounts office preferred.
