EIM (a division of SeniorsPlus) is Maine's leader in Care Coordination serving consumer's in the Brunswick area of Maine and assisting individuals in successfully staying at home. We have full time opportunities within a team environment to work out of our Lewiston offices. As an EIM Care Coordinator, primarily through telephonic care coordination, you will assist consumers participating in Long Term Services and Supports to achieve maximum independence and positively impact and improve their well being and health care outcomes through a collaborative process of assessment, planning, implementing, monitoring and coordinating with other health care professionals. Some travel and home visits are required.

Bachelors degree in Health or Social Services field and must have a minimum of 1 year community service experience. Preference is given to licensed or eligible to obtain license in social service (LSW, LSX, or LMSW) or health professional field (RN, OT, or PT).
