SeniorsPlus is looking for people who have a passion to work with older adults and those with disabilities. If you have strong computer skills, are able to work independently, and have a desire to assist consumers in receiving appropriate, effective and efficient services, this is the right job for you. EIM (a division of SeniorsPlus) is Maine's leader in Care Coordination serving consumer's in the Portland area of Maine and assisting individuals in successfully staying at home. We have full time opportunities within a team environment to work in an office setting or remotely from your home office. As an EIM Care Coordinator, primarily through telephonic care coordination, you will assist consumers participating in Long Term Services and Supports to achieve maximum independence and positively impact and improve their well being and health care outcomes through a collaborative process of assessment, planning, implementing, monitoring and coordinating with other health care professionals. Some travel and home visits are required.

Working knowledge of Long Term Care System, MaineCare, Medicare and Home Care Options required. Strong computer skills are essential. Multi-lingual capability is a plus.
