Care Coordinator - Portland area

EIM (a division of SeniorsPlus) is Maine's leader in Care Coordination serving consumer's in the Portland area of Maine and assisting individuals in successfully staying at home. We have full time opportunities within a team environment to work in an office setting or remotely from your home office. As an EIM Care Coordinator, primarily through telephonic care coordination, you will assist consumers participating in Long Term Services and Supports to achieve maximum independence and positively impact and improve their well being and health care outcomes through a collaborative process of assessment, planning, implementing, monitoring and coordinating with other health care professionals. Some travel and home visits are required.

Requirements
Working knowledge of Long Term Care System, MaineCare, Medicare and Home Care Options required. Strong computer skills are essential.

Education
Licensed Social Worker or a Registered Nurse or other licensed health professional, within the State of Maine, with a minimum of two years of community experience.  A working knowledge of the long term care system, Mainecare and Medicare, as well as Home Care Options required. Case management experience preferred.
