Position Summary:

The Director is responsible for the operations of the clinical integration team comprised of population health nurses, Senior Quality Program Manager, Clinical Integration Manager, Training and Senior Project Manager. Responsibilities include: collaborates with Population Health Nurse Supervisor and team to support clinical protocols and outcomes. The position will work with the VP of Delivery Systems Operations and VP of Provider Staff to execute on standard work across our Health Centers as it relates to our Risk Contracts and Quality initiatives. This position monitors appropriate utilization and administration of programs to ensure consistency of decision making, quality of service and cost containment.

The Director works closely with Practice Leaders, the Medical Management Department, and the GA team to help operationalize clinical integration efforts.

Key Outcomes:

Clinical Integration:

  • The director will lead a team to oversee value based contracts and drive profitability through development of a scorecard.
  • The director will develop strategic partnerships with commercial payers, employer groups, specialty groups, and community health systems such as Maine Health Management Coalition in order to drive the triple aim.
  • Collaborates with other departments in the organization to ensure their strategies are integrated and aligned with those of the department

  • Supporting accreditation and contractual obligations as necessary

  • Collaborates with defined owners to design and implements and oversees various nursing-led clinical programs, such as Coumadin Clinic, Opioid Clinic, etc, which support delivery of excellent quality of care and service to our patients.

  • Develops an overarching clinical support and nursing staffing strategy for the DS which ensures that all CS/nursing staff receive appropriate orientation, training and development.

  • Collaborates and coordinate activities with Health Plan and Delivery System leadership and departments to promote the Clinical Integration Team programs.

  • Manages Operations of department including but not limited to auditing of programs, population assessment and stratification

  • Monitors program evaluation metrics and measurements to ensure that targeted population health management interventions remain relevant and appropriate
  • Initiates, defines and analyzes target populations for complex care management and disease management programs. Performs ongoing measurement of outcomes, modifies programs to continue to improve performance
  • Monitors staff productivity and performance metrics and outcomes to ensure productive and efficient team that meets all service and timeline standards


  • 10+ years of professional level experience in health center and/or medical practice

  • 5+ years leadership and/or managerial experience

  • BS Nursing Degree from an accredited school of nursing preferred

  • Ability to write executive reports, peer reviewed manuscripts, and power point presentations, as well as the ability to present findings in internal and external settings.
