Health Home Coordinator
serving the child population.
Use your communication skills to provide patient, caring, and compassionate service to our consumers. As a Health Home Coordinator you will help others advocate for themselves by developing a holistic plan that:
Identifies strengths and sets goals
Improves whole health and wellness
Facilitates coordination of care
Develops natural supports
Provides linkage to community resources and services
Collaborates with families and providers
Develops systems of support to help manage symptoms
The Behavioral Health Homes model is a team approach to integrating care across all aspects of physical and mental health. The Health Home Coordinator will work with a Clinical Supervisor, Nurse Care Manager, Psychiatric provider, and a Peer Support Specialist to support the success of our consumers in leading a self-directed life and reaching their full potential. Through the use of a shared Electronic Health Record; participation in team meetings and case reviews; inter-agency and intra-agency meetings; and group and individual supervision, the Health Home Coordinator acts to link individuals to community resources that support their unique needs and goals.

Minimum of Bachelor's degree in related field (or unrelated field with a minimum of one year related experience in the field)--OQMHP eligibility is required. Valid driver's license and computer proficiency are also required. Duties require knowledge of and experience in working with children with special needs and the ability to adapt to the individual needs of a child/family.
