The Nurse Care Manager will work with a Clinical Supervisor, Health Home Coordinator, Psychiatric provider, and a Peer Support Specialist to support the success of our consumers in leading a self-directed life and reaching their full potential. Through the use of a shared Electronic Health Record; participation in team meetings and case reviews; inter-agency and intra-agency meetings; and group and individual supervision, the Nurse Care Manager acts to bridge the gap between physical and mental health.
