Onward Transitions

Onward Transitions is a privately owned and operated short term non-residential living service that supports emerging adults (members) ages 18-29, living independently in their own apartments in Portland, Maine. Member are bright, motivated, and supported by their families in their pursuit to gain autonomy and live independently. Member challenges including mild to moderate anxiety, depression, and/or executive functioning challenges. We are not a recovery program, nor do we provide acute care. From a family styled home on the border of USM-the Pine House- and an office in the Old Port, we practice generalized living skills and provide meals, a safe community, therapy and social, career, vocational and life coaching, apartment consultation, social activities, and community outreach. Our members are referred to us by educational consultants and mental health professionals from across the country. They work with us for an average 12 months


 

Essential Functions:

  • Serve as a positive mentor to our members.

  • Ability to work as part of a team and autonomously.

  • Work closely with a team of therapists, coaches, and staff in a treatment team format and participate in monthly supervision.

  • Meet individually with members as needed (minimum of weekly) for coaching around treatment goals as defined in their individualized Family Agreement (i.e.- education, etc.).

  • Develop comprehensive experiential learning programs with Program Director and Clinical Director.

  • Serve as primary experiential conduit for members to explore and achieve academic goals (i.e.- planning, registration, accountability formats for checking grades, etc.).

  • Design curriculum and lead monthly life coaching groups.

  • Maintain coaching group resources for assigned topics around our Family Agreement.

  • Plan and lead twice monthly Saturday group activities.

  • Manage apartment consults as directed by program coordinator (minimum every other week).

  • Cultivate relationships with educational service resources (colleges and universities, adult ed., technical training, etc.)

  • Maintain daily milieu notes for members

  • Assist with daily operations in the Pine House and Hay Suite office space (coverage, meals, cleaning, light yard work)

  • Demonstrate a positive attitude and flexibility

  • Demonstrate initiative in helping to problem solve members’ needs

  • Other duties as directed

 

Requirements:

  • Must be 25 years or older

  • Bachelor’s degree in a related field or Associate’s with significant and relevant professional experience

  • Must possess a combination of related education and work experience

  • Must submit to a criminal records and driving history check, have a valid license and insurance

  • Must have 3 professional written letters of reference

  • Must be willing to maintain appropriate professional boundaries with members


 

Preferences:

  • Experience with executive functioning coaching

  • Experience with college application process

  • Experience working with young adult/adolescent populations in educational or mental health environments.

  • Knowledge of Portland resources and recreation opportunities

  • Possess a breadth and depth of talents and interests, appropriately shareable with our member community

  • Some tech/G-Suite/Social Media savvy

  • Some comfort in meal preparation and cooking with small groups

 

This is a full-time position with health benefits and earned time beginning after 90 days. Salary is competitive. Some meals are provided.
