Caledonia Home Health & Hospice is seeking a full-time Licensed Practical Nurse (LPN) to join their devoted clinical team. This potential team member should be a compassionate, team-oriented individual who desires to provide direct patient care to clients in their homes, under the supervision of a registered nurse. Responsibilities include following an established plan of care, providing treatments, and working collaboratively with the members of a team to achieve the highest level of coordinated care for clients served in Caledonia and Southern Essex counties. Candidates must have a current Vermont LPN license. One year of prior clinical experience is required, preferably in a home health setting.
